1. I had a similar issue with a 2x anamorphic lens (although it was the quite bigger Isco-Gottigen Anamorphotic Kiptar) and I decided to poke around and see if I could solve the issue myself. The culprit was calibration: the front and rear element of the lens were not properly aligned. Judging by your footage, I'd say that the lens is just not properly calibrated. If you feel adventurous you could try to "operate" on the lens yourself, in my case all that was needed was a bunch of screwdrivers and a couple hours. I suppose you could also bring it to a professional and have the job done right, it's just the matter of finding someone qualified (and unfortunately I can't help you with that!). Don't take my word for it though, maybe other members have better solutions/thoughts. I hope it helped nonetheless.
2. If I may: I'm not sure it is the one he used, but somebody posted a link to this 4.5k film scan for grain. You can find some more info on the pv topic there: http://www.personal-view.com/talks/discussion/3665/film-grain-overlays-.-.-.-a-trick-for-getting-better-4k-with-anamorphic-from-your-1080p-camera (I hope I'm not breaking any rules by linking to that topic!).
3. I'm having the same issue (but with the DP4). The 2.35:1 scale preset just doesn't cut it, and messing with V/H "customscale" didn't help either. EDIT: I opened a support ticket with SmallHD suggesting they include the 3.56:1 (3.55:1?) scale option in a future firmware update. We'll see what they respond.
